PREVENTATIVE HEALTH REMINDER SYSTEM

This practice is committed to preventative health care. Your doctor may seek your permission for you to be included on the practice reminder system.

If you are included in the system, from time to time, we may issue you with a reminder notice, offering you preventative health services appropriate to your care.

As this is a voluntary service, please notify your doctor or practice staff if you wish to take advantage of this service.

DISEASE REGISTERS

If you wish, this practice can submit your data to any disease-specific registers (e.g. Cervical, Breast, Bowel Screening, etc.) to assist with preventative health management.

If you desire this service, please inform the practice staff as there is a Consent Process to establish such submissions.

FEES and BILLING

This practice bulk bills your medical treatment in accordance with the Health Insurance Bill 1973 and the Health Legislation Amendment Act 1983 (Medicare).

Workers Compensation Consultations and Treatment are NOT included under the above fee conditions.

MEDICAL TESTS or PROCEDURE RESULTS

Your doctor will advise when the results of any medical test or procedure are expected to arrive at the practice.

Please call the practice to be informed of your results and what, if anything, the results may mean regarding your continuing care.

PLEASE NOTE: Administration staff are NOT permitted to discuss with you, any details of the results of any medical procedure or investigatory tests.

For such details, please consult your doctor or a Practice Nurse.

HOME and OTHER LOCATION CONSULTATIONS

At the discretion of the treating doctor, home and other location consultations are available for regular patients of this practice, whose condition prevents them from attending the practice rooms.


OTHER POLICIES

OFFENSIVE BEHAVIOUR

Under no Circumstances, will any of the following be tolerated at any time:
Rudeness, Aggression, Intimidation, Foul Language, Violence or Physical Threats to staff or other patients.

TELEPHONE ACCESS TO DOCTORS

Doctors in the practice may be contacted during usual practice hours.

If the doctor is with a patient, the practice staff will take your details and a short message, then based on your information, they will advise you of when it is likely that the doctor will return your call.

Please rest assured, in the case of an emergency, your call will always be connected to the doctor as soon as possible.

CULTURAL IDENTIFICATION

To enable this practice to better provide for your individual health requirements, please inform our staff if you identify with a specific cultural group (i.e. Aboriginal, Torres Strait Islander, or Other)

ELECTRONIC MESSAGE POLICY

This practice does allow for patients to communicate via Fax and Email only. All other methods of electronic communication {SMS, MMS, etc.} will be rejected.

Please remember, the most effective method of contact between you and this practice is the telephone.

If an Interpreter Service is needed for you or a family member, this can be organised. Please make this request known when making your appointment.
